I cant use jotform.. please do something

I see that the account is auto-suspended by our anti-phishing system which is strict to scan each form to calculate the probability of phishing activities. I see that your JotForm titled "Account Information" is collecting username and password which is not allowed with JotForm.

It is not allowed to create login forms or collect sensitive information like Credit card numbers, etc through JotForm. Such forms are considered as phishing forms and associated accounts are automatically suspended. Please refer to our Terms of usage for more information on phishing.

It is required to accept our terms of usage and to delete the guilty forms in order to activate the account. Please confirm so that we can delete the guilty form and activate your account.

I see that your forms are still associated with a guest account and you can sign-up for a free account so that you can manage your forms easily.
