Transferring input data to existing PDF document.

    Is there any way to have my patient's fill out a form and make it so that the data they input can fill in the blanks of an existing PDF file?  The alternative is to have the patient fill out my PDF form directly on Adobe.  My vision was something like that but using jotform as a platform.  

    I've attached a PDF file of my paperwork.  I was thinking about just creating a online form for the first two pages but if I did it for the entire document that would be a bonus.

    Hello mikelnewman,

    You can upload your existing PDF to JotForm through this link: https://www.jotform.com/fillable-pdf-form-creator/

    On the page, click the Create a fillable PDF form button. and select the 2nd option (at the center)

    This will then allow you to upload your PDF file. JotForm will then convert your uploaded PDF to be able to receive submissions.

    You will then have to download the PDF file from your My Forms page as this new PDF file will connect to JotForm once you make a submission. 

    Note: Just keep a copy of your original PDF for back-up purposes.

    I have done this and I see the PDF file converted over. I am now having
    trouble filling it out and submitting it. Can you provide me assistance
    with this? Is it possible to fill out a PDF on a mobile device and submit
    through that?

    If it's not fillable please use Adobe Acrobat Reader or Foxit., PDF forms are not mobile responsive.

    There is a service that will allow you to create a mobile form that can be filled out on smartphone, tablet and iPADs. You can map the submitted data to your existing PDF so it fills out automatially. You can program the competed PDF form to be emailed to you when the user submits the mobile form.
