I have been trying to send form data to a mailchimp list to kickstart a workflow. When I test this with a completely different email and test data, nothing happens when submit is clicked. It isn't sending data to mailchimp. Why is this?

I did several tests from my form and from your cloned form and the data is going through. Wha you could do is to check your custom fields if their type is correct on mailchimp's side, meaning phone number is type phone, name is type text etc. I found this useful article from Mailchimp about how to do that: http://kb.mailchimp.com/lists/managing-subscribers/manage-list-and-signup-form-fields
Also, you could try to re-integrate and see if that will help.
