I can't find my form to edit it and it is not receiving the emails.

We have a registration form posted at http://grantwoodcc.com/connect/workshop-registration which is displaying the form correctly and is allowing me to submit info as a test, but I don't get the email, even though I have in the past. I am worried that many have registered and I have no idea how to track it down. I tried finding it in my forms as well as searching through my browser history but to no avail. Please help. I tried to copy it below as well, not sure if that helps.

The form titled Family Marriage and Parenting Workshop Registration was created on a guest account, which is why you were not receiving any emails and cannot find it in your account. Guest accounts are usually created when a user creates a form without logging in to their jotform account.
