Hello,
I made a feedback submission page for my online community. When I went to preview the link, the form is disabled. My guess is this is because the term "username" is used. This is in use because some of our members do not give out their actual name and only want to be called by their username alias.

To enable your form, just enter the preference wizard. Under the GENERAL tab, you will the the option STATUS. Just change this from DISABLE to ENABLED

I'm also now replying from my personal account. I signed out of our guild account and when I tried to sign back it in, it said I was suspended for phishing. Again, I think the anti-phishing scan saw that I asked for username, and it triggered the disable response.

I see that the account is auto-suspended by our anti-phishing system which is strict to scan each form to calculate the probability of phishing activities. I see that your JotForm is having a field containing username which is causinig the account to suspend.
It is not allowed to create login forms or collect sensitive information like Credit card numbers, etc through JotForm. Such forms are considered as phishing forms and associated accounts are automatically suspended. Please refer to our Terms of usage for more information on phishing.
I have activated your account and you should be now able to view the forms normally. Please remove the word username from the field to avoid any further suspensions.
