How can I use a converted PDF form in Jotform?

I've already addressed this question before with a generic "how to" response.  So far, I've taken a PDF form and converted it over to a fillable form.  I want to take the next step and put this "fillable form" on my website or at least have a link for people to fill out.  I've tried getting the link from the publish button but that doesn't seem to work.  Please help

Please note that it's a PDF file that you will need to uploaded to your server if you want to display it on a webpage.
