I own a promotional product company and had set up an account to create forms for proof approval and sign up and things of that nature. Well, then one day I went to log into my account and it said that it had been suspended for suspected phishing.....I responded to try to get everything cleared up multiple times and nobody ever contacted me or anything so I just created a different account. When I opened it up today, i noticed that I was no longer logged in, strange because I stay logged in and was still logged in on other sites that I had up. I thought I had logged in and it said that this account had also been suspended, BUT because I have had to create so many different account I now CANNOT remember what my log in info was, whether that was my new account or my old account. The problem with this is that, these forms are how my clients get their artwork over to me, how they approve their proofs, and how they request new products. I am currently producing products for this coming football season (which starts in less then 2 weeks) and I have proof approvals sent out to numerous clients to which I can no longer access their approvals, making it impossible to produce their products, and potentially leading to chargebacks for unreceived products! With that being said it makes it hard to conduct business with things like this continuously happening. Please contact me so that I can give you WHATEVER it may be that you need so that you will stop doing this to me (ie federal tax id number) whatever I need to do I will do but I cannot have this constantly happening. The account in question currently is the one with this form https://form.jotform.com/62024815135145

The account that contains the form you have linked belongs to the account with the username cwest. I would like to inform you that your account named cwest is not suspended. You should be able to use your forms there without any issue.
If you are unable to login, you can reset your password from the login page's Forgot Password section. However, it appears that the email address connected to the cwest account does not exist (adzrussponso@********.com). We will need to change the email address on your cwest account so that the password reset instructions will be sent successfully. If you want to reset your password, please provide us with a new email address that you can use for that account and we will change the email address accordingly.
I will wait for your response.
