All of the forms I try to create say they're disabled. From what I can tell reading other complaints, my profile has been blocked. Was I violating any terms by using my business email? I really would like to have even a single form work to accept payments on my sites. This form isn't even for my business site that has special payment requirements.

Your account was autosuspended by our phishing detector due to violation of jotform's terms. Please note that collecting login credentials such as passwords is strictly prohibited. The form you created violates this rule so your account was suspended.

I have now removed the password field on your form and reactivated your account. You should be able to use your form successfully now. To avoid getting suspended again, please refrain from collecting login credentials, social security numbers, credit card info, and other sensitive personal info.
    Regards.
