When downloaded as a pdf it shows up all funky / blurry. How can I correct that? I would love to have a downloadable form available. Thank you!

I understand that you are trying to download the form to a fillable PDF and the PDF generated is not looking good. I have checked your JotForm and see that there are several images with matrix fields. Unfortunately, we have some limitation to process the form to a fillable PDF particularly with some fields like Header, Text field and Images.
