I created a form and got a link....when i go to my forms it isnt there, but when i click on the link it opens the form. Where can I see this to see submissions

Based on the History logs, it shows that you updated this form.
The URL that you provided is a custom URL that uses the account username and form title. However, it seems that you changed the form title after cloning it which is why the custom URL is not working.
In My Forms, you'll see the correct form which is titled as "Clone of PLMS BOYS BASKETBALL ORDER FORM". Please check the submissions of this form.
