Fields not appearing in the submitted form

    Hi There!

    My Jotform has mandatory fields but when I receive the submitted form in PDF, it does not contain certain fields. This is creating a real problem. Could anyone please suggest a solution. I am clueless on this.

    The box that does not appear is a configurable list box containing name and other fields. I have attached a snapshot of the PDF file where indicating where the field should have appeared.

    Any help in this regard will be greatly appreciated.

    I cloned your form and did a test submission. I received the attached PDF file and I was able to see all the fields. Here's a screenshot:

    In order to fix this problem, please try resetting the design of your PDF using the PDF Designer. Here's a guide:

    1. In My Forms page, select the form and then click the "Submissions" button.

    2. After that, click the "PDF" button and then select "Customize PDF" option.
