What happened to my form?

I worked on a form for several hours, saving as I went, then unexpectedly had to close and restart my computer. Now the form is not there. There is no url or screenshot available.

Is that the right form or was there supposed to be another one?

It's back now. I wish I had taken a screen shot while it was empty. I was able to get to it through history, but it didn't display under my forms. Oh well, it's good now. Thanks!
