Why did I get suspended and what policy did I break

    this is a huge inconvenience. I have 50 people that need the form I put together and now they can use it from our website. I need to know what policy I broke and what is the problem with my form. I'm 2 seconds from switching to another form company. If this form worked well we were gonna have a paid account but now I'm very uncertain
