Why the message "This Site Can't be Reached" is shown when submitting a form?

Hi Guys,

When I fill out and submit the form found here - https://form.jotform.com/62303832535148 I get an error message that says "Safari can't open the page" or "The Site Can't be Reached."  I've cleared my history and cache and have tried two different browsers, with no luck.

I'm sure I've got something configured wrong, but I can't seem to figure out what.  Any guidance would be much appreciated.

This may be a temporary issue, it should be related to the internet connection where the form is being submitted as well as I have tested your form and I got the Thank You page without any problem, please see the screenshot below: 

Please try submitting again, if possible try from another PC, if the issue still persists on your end, please let us know.
