How is the information from the forms captured. Is there an auto reply -if so does it have to come from my email address

Form information is captured into your JotForm account and can be viewed directly from there:

1,000 monthly views means your forms can be loaded up to 1,000 times.  Each time the form is loaded, that counts as a view.
