How to remove form from Facebook tab?

My link for FB is faulty and I cannot remove it or modify it.

How can I change it?

I am not sure if I have understood your question correctly. Do you mean to say that you have embedded your form in Facebook tab and you now want to remove it?

Here are the steps to remove facebook tab:

#1. Go to facebook app page.

#2. Click on the "Settings" tab.

#3. Click on "Apps" link form left menus.

#4. Click on "x" button of JotForm app and also confirm by clicking the "Remove" button. Please check the screenshot below:
