We are having issues with our form at the very end, when a user tries to submit credit card payment information. The form is completely filled out, they hit the "Next" button, and then the word "Next" becomes "Please Wait..." and freezes there. Is this an issue with the form or with our payment system?

We didn't experience any credit card issues at this time. Did you try to process a payment on a form directly by any chance?
I'll try to do some more tests and get back to you as well if I'm able to find the cause of the problem you're having.

Authorize.net would like to know the URL this is being sent to. Are we using Advanced Integration Method?

I'm sorry, but I'm not sure which URL you need? This integration is done through API key integration.
Please be mindful that all information is sent to Authorize.net, meaning nothing is stored on JotForm. Also, you can test the integration in sandbox mode, for that please follow this guide: https://www.jotform.com/help/221-How-to-Test-Authorize-net-Payment-Tool-on-Sandbox-Mode
