Why my account is disabled?

When I click in the preview button to check what I am doing, it seems the form is not working properly. Pops-up a message saying my form was not found.
What am I doing wrong?

Your account is disabled because of our anti-phishing system that detects suspicious forms and disables them, and the problem is in word 'password' that you have in your form and you titled your form like that as well:

Collecting passwords through our forms is against our Terms of use.

I removed that title from your form and reactivated your account.

Please avoid collecting such data in our forms to avoid being suspended.
