How can I have a single checkbox with no label?

I want a single checkbox with no label.

I delete the 2nd & 3rd checkboxes.

I delete the label field.

I have attached an image of how I need it.

You can place a space as your one and only option. I did it for you. But looks like the checkbox is automatically checked and I find no way to uncheck it by default. You may consider switching both.

Make "Use Regent Park Address for Correspondence" as the option without the label. See this image:
