I requested a unique form as I did not see one available - how does your process work ?

Yesterday I requested a new form - one that is like your product lightbox but when selected, the selection goes to a form that has a window where the selection shows up and the form then can be purchased as a downloadable .pdf.

Where did you request the form? IF you have it requested over the forum, did you get a reply from my colleagues? I can't seem to find a log for that so maybe you have requested the form using a different email. 

Do you also have a sample form where we can check how that form works? Based on the given description, I think you would like to have a preview of the selected products. We have a widget that would allow you to preview answers, you might want to check that.
