On submission we get a blank screen. Submissions are being saved into database but the user does not get a thank you page or email sent. Please help, this form is in constant use.

I checked you form and I was able to submit it successfully. Upon submission, I was redirect to its Thank You page.
The issue seems to be fixed now. Do let us know if you need further assistance.
