My form has been disabled for no real reason

    I look on the forum and found that this had happened to a user. He asked what had happened, and the answer was:

    "Your account has been suspended because of the way that is requesting a username and password (Login Form). To restore your account, please delete the form or tell us why you have to create a form to request a username and password."

    I do not requested name or password for anything.
    My form has two steps and asked them to create a password with three numbers to confirm the second stage they were themselves.

     
     

     

    So I do not understand what could have happened. I exclude the question, but nothing happened.

    Please do note that this is why you're account has been disabled, also avoid using the words password or username in your forms, although you're not asking for this info our anti-phishing system may catalog this as a phishing form.
