Why my Email field and Full Name field does not show up in preview?

The Email field and Full name Field is not showing up in my preview. What do I have to change in order to see it when I send people the link to this Jotform.

I checked your form and the problem is in Conditions that you set. They all have missing fields:

You should fix fields in Conditions in order to show them properly. If you want to test just disable conditions and it will work normally.

Why can't I enable the conditions fields that I have?
I need their name. And email and who their coach is. I can't have them submit the form and not have those answers. It's the reason for the form. How can I make changes around those needs?

I removed the conditions which were all broken anyway and not necessary on your form.
