How to have Quiz form that have conditional feedback option

However it is a bit more complicated than that!

I want the user to be able to complete the quiz online. The quiz is yes/no answers. From the answers i want to develop a report with answers that are conditional on their answer i.e. there is feedback for a 'yes' answer and different feedback for a 'no' answer for each question.

I also want to be able to add in some tailoring for each report i.e. a personalised summary.

Is that possible?
