Why can't I send emails to intended recipient

I'm creating a time off form.  The requestor get's an email = great.  It goes to an ADMIN = great.  BUT THE MANAGER who needs to receive the request, CANNOT RECEIVE THE FORM (AND I CAN"T FIGURE OUT WHAT I'M DOING WRONG)  FORM #https://form.jotform.com/62285669451970
