Campo de selección, se puede direccionar a otro formulario si la persona lo selecciona?

Hola, tengo un formulario con campos de diferentes lugares, pero quería saber si se puede hacer que, al seleccionar una de las opciones de lugar, se despliegue un formulario distinto a el de los demás, con diferentes horarios en los campos y diferentes puntos a seleccionar.

Ok, I have been trying to add, whenever someone pick a determined place in the options that are displayed in one of the squares, there is one option that i would like to display different options than the other onees, for example, the timings, the extras, so when somebody clicks that determined place, they have special options for that place only, like, if they click that option, another form would display instead of the same one for everyone.

Could it be done?

You have one show/hide condition in the form. You can do the same for the other fields.

I tried it already, but what i need is that when the restaurant is equal to passion, then the fields of schedule, number of persons and extras , change... Like special fields just for that restaurant. Is it still possible? 

You can duplicate the fields and put it in the section. If you have 10 restaurant items, you'll have 10 sections in the form.
