How can I have my text show on a PDF?

Hello, 

On one of my forms, Camp Roger Paperwork 2017, I have a paragraph of wording and then the parent's sign on the bottom. But when I pull the PDF of their answers, none of my headings or paragraphs of text appear on it. Is there anyway to make that a possibility? So that I can actually see what it is they are signing and agreeing to?
