Unfortunately, we don't have a feature that would allow you to do this in the form. You may consider uploading the excel file to your Google Drive account and embed it in your form. You can use our iframe embedder widget to embed the file in your form:

If you really want to link the two, you may consider hiring someone to program it in the form. You can get the form source codes to add the custom functions in the form.
