Can I do a table with Checkboxes that will allow multiple answers

    Can I do a table like this (see attached)? The employee needs to be able to select multiple answers so it needs to be check boxes (not radio buttons)

    I think the Matrix table field is the one you need. You can find it among the Survey group of fields.
