one of our staff that is testing it is still having issues on both chrome and safari. I've attached screenshots.

First of all, apologise for the inconvenience. That issue should be fixed yesterday by our developers so quickly. I’ve cloned and tested your form on Safari and Chrome. I am able to submit your cloned form without any issue.

I'll get our staff member to retest it tomorrow (it's now 9:30pm here).
I'll post back here if it continues to be an issue.
