Form gone after clicking Themes?

i was in the middle of creating a long questionnaire, and i clicked on themes and it was gone. can i recover it somehow?

To make sure that your form will be saved properly. I strongly advise that you register an account using the same email address so that you can easily edit your forms anytime. 

Please register and account, once done, you should automatically see the form you created on the "My Forms" page.
