I cloned a form and then got rid of the name field to create my own. Now none of my additional fields are showing. Help?
I've had this happen on several different forms. The issue *mostly* resolves itself by the time support can reach me, but I'd love to know how to stop it from happening altogether.

Hello,
As i see , you have a hidden calendar and a hidden textbox with a title in your web-page. When i open its CSS codes , looks like it is configured as "display : none"
When you clone a form, you need to arrange their CSS codes as "display : true"

Your form https://form.jotform.com/62716106678158 you have cloned have existing Show/Hide condition rules that was affected when you delete some fields that were associated to the condition rules.
To fix the issue, simple delete those Condition rules that have error on them. Then, save the form.
It should resolve the conflict on the form conditions/rules due to the error on deleted fields.
