Form Not Found. This form is disabled.

    I keep getting this error even though the form has been published and I made sure it was enabled in the settings. Here is the link to the form: https://form.jotform.com/62921594750158

    Hello,

    I've checked your account and your forms, I've seen that your account has been auto-suspended due to phishing. 
     
    You have wanted password from users in your form named "1st CRT Group - Teaching & Education Registration FORM" as you can see the screenshot below. You are not allowed to do that according to our terms of use.
