I added a requested password field without realising it would trigger the auto phishing

this is actually a form for people to sign up for a wireless account, but I've removed the offending field

You are correct, the password field triggered our phishing tool to suspend your account. You are not allowed to collect login credentials, credit card information and sensitive details here in JotForm. Please check our terms of use.
