How to create a check list with sub menus

    I want following functions;

    applicant can check multiple languages and choose level of knowledge for each language. I also want other button in the list if that is selected then they can enter the language and the level.

    For example

    English-  Functional, Intermediate, Excellent

    Tamil-  Functional, Intermediate, Excellent

    Dari-  Functional, Intermediate, Excellent

    Hindi-  Functional, Intermediate, Excellent

    Other- ... Names...

    I want applicant to select one or more language and click at their level of knowledge. If that language is not on the list then to click on other and then type the language.

    how I can create such list?

    Hi,

    I have navigated through our widgets but none of them actually works the way that you'd like them to. The best way available seems to be using our Check Box and Drop Down fields. 

    Please see how you can do it:

    1. Create a Check Box field:

    You can determine your choices like this. You can also specify an "Other" choice where your users will be able to type. 

    2. Then you can create hidden Drop Downs that are conditionally linked to the Check Box field.:

    You can create as many drop downs as the choices in Check Box fields. Please take a look at my test form that I just created:
