Also, these selections need to go into PayPal. The first section needs to go into the custom field. The second section needs to go into the "Item Title" field. How does one accomplish all this? Thank you!

You can make the selection from the "Please choose the giving level and gifts you wish to receive." field to become the field name of the payment field by following the instruction in the screenshot below.

Below is a screenshot of how it should work.

Unfortunately, the workaround above doesn't work for the "Description" of the amount field and I'm afraid it is not possible to update that base on a selection from another field.
