Using the same form on multiple websites, is there a way I can change checkbox options for each website?

    I'm looking to use one form for multiple websites. However, each page will have to have different content for their terms and conditions which a visitor is required to mark the check box to proceed.

    I've looked at my embed options and I'm not sure If I can have the actual checkbox option language in the code so I can manually change them for each site?

     

    Example:

    DomainA.com - there's a check box stating you agree to DomainA's terms....etc.

    DomainB.com - I want to use the same form, but the option for this checkbox states you agree to DomainB's terms.

     

    Hope there's a way to do this, thanks in advance.
