I have had this form on my page forever. Just today I decided to change out the ADDRESS field to one that is itemized (street, city, state, zip) rather than the box (which is what I have had all along).
The problem is that in testing the form, the address is accepted but when I receive the notification email the address does not show up there. It is showing up on the Jotform platform. Any thoughts?

The problem is in the name of the address field, the name is:

I change that for you to match the field's name, so please give a try and let us know if the issue persists.

So it was the addition of the "17"? Funny I looked at "Notification" figuring it had to be something there and totally missed that.
