I need help creating a survey that people can use to submit invoices

I want to create a jotform that people can use to submit invoices. I've looked through the survey templates that are already out there, but I haven't seen any I liked.

I want it to be pretty straightforward with a list of expense and the person filling out the form just puts the expense amount and receipt. 

For exmaple, 

1. Day Rate -

2. Per Diem -

3. Mileage -

From there, the person would just fill in the amount in a box and attach receipts.
