image not showing on PDF submission

Hi there,

I have a logo at the tome of my form which does not show up in the PDF attachment that the user receives.  Can anyone advise me on how to fix this please?  I have attached a screen shot to show that I just get an empty box showing up.

Ideally i would love for the PDF to look exactly as it looks on the 'submissions' screen, is this possible?

You can reset the design, so you will get the default design of the PDF submission, example:
