Which fields on your form have been rearranged?
And how were they before?
You can try checking your form revision history and reverting your form to a version before it was changed.

I'm quite certain no one at JotForm did anything to the form :) I was building the form in v4 and changed back to v3 and it showed up all in order and the formatting was correct. I don't know if maybe there are some quirks in v4 that need to be addressed. I did find it pretty easy to use and even preferable when I moved back to v3.
I didn't think about checking the form history, but will do so if it happens again. Thanks for the suggestion. I'm good with this issue now.
