We do apologize about any inconveniences you might be experiencing though none of our team members would have done this without your permission so perhaps there's a logical explanation and we'd be more than happy to assist you in getting it back the way it was.

Which fields on your form have been rearranged?

    And how were they before?

You can try checking your form revision history and reverting your form to a version before it was changed.

I'm quite certain no one at JotForm did anything to the form :)  I was building the form in v4 and changed back to v3 and it showed up all in order and the formatting was correct.  I don't know if maybe there are some quirks in v4 that need to be addressed.  I did find it pretty easy to use and even preferable when I moved back to v3.

I didn't think about checking the form history, but will do so if it happens again.  Thanks for the suggestion.  I'm good with this issue now.
