You have exceeded the monthly form views on your account. I see you have as Silver subscription which form's views limit is 100,000. Your for views for this month is 100,296. So you will need to wait until the counter resets on December 1st, or upgrade to Gold plan.
Another option would be enabling JotForm Footer on your form in order to permanently lift this limit: https://www.jotform.com/help/355-Form-View-Limits-What-is-it-all-about
