How can I upload a form's URL? It won't accept the website I used

    how can i upload a form someone else has created and use it as my own? I see some other doctors are using and would like to use the same one

    I noticed you included a link to a PDF file and this appears to have some form elements in it.

    BUT, this doesn't necessarily mean that the PDF file you included was created through JotForm. And even if that's the case (which I highly doubt after seeing the PDF), you shouldn't use someone else's form without the owner's consent.

    If you wish to use that same PDF file on your website, try contacting the owner of the PDF file.
