Please help me fix the appearance of my form

Hi,I tryed to use letter box theme and then tryed to cancel it (because I couldn't resize input box accordingly), now any imput field is messy and I can't bring them back to normal.

Please see how you can get your form back to its normal stance:

1. Click "Themes":

2. Click the "Default Theme" on the right:

Then it appears as expected:
