Please take a look at the autoresponders for my EFT Level 1, Level 2, Level 3 forms (same problem for each). I added an image logo, which looks great on the autoresponder setup, but shows only a broken link for the image on a test email. Can you see what the problem is?
I have cloned your form and was able to reproduce the issue you're having.
This is caused by the file name of the uploaded logo/image in the email body.
I checked it by viewing the source code of the email body and I see the spaces in the file name.
These spaces are not recognized in the email when it is being sent.
To fix the issue, please replace the "spaces" with %20.

I just replaced the spaces with dashes in the name of the image and uploaded it again, then used the new name assigned by jotforms.
