Form error on website

I still haven't heard back from anyone regarding our issue with our forms on www.greatlakeslabrescue.org

When you submit the form, an error appears in the console log.

Uncaught ReferenceError: $$ is not defined(…) submit.jotform.us/:25

Try replacing your oEmbed form with your forms iFrame code:

Remove the currently embedded form and replace it with the iFrame code for your form.
