Form not working in my site properly

Hi, I spent hours creating a form that works perfectly from within the builder; including travel distance calculator and  reservation check-in/ check out fields. Once everything was as I wanted it I embedded it to my site. Unfortunately, once it is embedded the form doesnt work properly. The travel calculator, the check-in/check-out and the terms and conditions fields are all malfunctioning. 

Are there any solutions you can offer??

I did a test submission to your form direct link and it is working fine. Unfortunately, I can't open the Page URL you provided. To my understanding, your form is not working properly after you embedded it to your website. 

Please try this, re-embed the form using iFrame method. Here's the guide: https://www.jotform.com/help/148-Getting-the-form-iFrame-code
