How to make forms Mobile friendly (responsive)?

What is the best practice for ensuring your forms "fit" correctly when a customer accesses your form from a cell phone?  Some of my forms have various widths and do not nicely appear on a mobile device vs. when accessing from a tablet or on the web from a computer?
