How con a create a simple registration/r.s.v.p. form

Once I've created the rsvp form, how do I make it available to the invitees?

I want only basic information:

1. Title

2. Name

3. Church or Organization Name

4. Location/Address

5.Contact information

    a. Phone

    b. Mobile

    c. email address

    d. website

6. Number or guests attending and their names

How can I access the information once entered? Do you host the site or do I obtain my own website? How easy is it for the invitees to access?
