Page skip not working properly with the conditions on my form

I have a radio button that asks users "Have you filled out our registration before?" and when the answer is "Yes" I want it to take users directly to a screen that asks "Would you be willing to fill out our photo, video and sound recording release," however with the condition I'm using, it takes users instead to a blank page. I've tried any number of conditions but none of them seem to get me where I want to go.

I have checked your JotForm titled "DRAFT UPDATED STANDARD Up Down Circus Registration" and could not find the specified fields "Have you filled out our registration before?" or "Would you be willing to fill out our photo, video and sound recording release,". I believe that you are referring to the following fields.

Since you want to change the page based on the selection, you may consider using the Skip to/Hide a Page condition instead of Show/Hide fields condition.

Please go through the guide below that can help in setting up the conditions using Skip pages.
